Thank you for your donation! We now invite you to submit a short story about a friend or loved one who served in the military. This could be a memory, a lesson they taught you, a heroic moment, or the impact their service had on your life.

Ideas of what to include:

Who they were and your relationship to them
Where and when they served
A specific story, memory, or reflection
How their service or character made an impact

Tips for Writing:

Keep it personal and heartfelt.
Use clear, respectful language.

I authorize Elara Caring Hospice Foundation ("ECHF") and its affiliates to use and disclose certain information about me (or that of the individual I am acting on behalf of), as described below. 1) Information to be used and/or disclosed ("Information"): ECHF is authorized to use and/or disclose name, title, portrait, picture, video image, photograph, or any reproduction or likeness of me (or that of the individual I am acting on behalf of) or quotation of my remarks. Such use or disclosure includes but is not limited to all printed and digital publications. 2) To whom Information may be disclosed: ECHF is authorized to disclose my Information (or that of the individual I am acting on behalf of) to the general public. 3) Purposes of use and/or disclosure: I understand that my Information (or that of the individual I am acting on behalf of) will be used for informational, educational, marketing and/or promotional materials regarding ECHF. 4) Ownership: I understand that any materials that use my Information (or that of the individual I am acting on behalf of) are the property of ECHF and will not be returned to me. 5) No Compensation: I acknowledge that I will not be entitled to receive any compensation related to the use or disclosure of my information (or that of the individual I am acting on behalf of). Furthermore, I waive any right to royalties or other compensation arising from or related to the use of my Information. 6) Modification of Information: I irrevocably authorize ECHF to edit, alter, copy, exhibit, publish or distribute materials containing my Information (or that of the individual I am acting on behalf of) for the purposes noted above. I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my Information (or that of the individual I am acting on behalf of) appears. 7) Hold Harmless: I hold harmless, release and forever discharge ECHF from all claims, demands, and causes of action which I may have by reason of this Authorization. 8) Understanding: I have read this Authorization before signing below and I fully understand the content, meaning and impact of it.
